Frontline Heroes: Request Form

The Million Mask Challenge is a volunteer network that makes cloth face masks, surgical caps, and other items to fight COVID-19. Our priority is to support frontline health care providers. We also donate to help other frontline heroes, including hospital support staff, police, firefighters, paramedics, food pantry and shelter volunteers, critical government employees, and other frontline workers, as our supply permits.

We are responding to the pandemic in Virginia, Maryland, and Washington, DC.

Masks are donated — not for sale. Please use this form, rather than soliciting in our social media groups. Thank you!

    As a 100% volunteer run organization, we appreciate your time to help us by filling out this request form. We are careful with how we handle donations of our craftivists' hard work. The retail value of our donated items is about $7-$20 each. Everything has been made with love. We will be in touch with you soon to talk about your request.

  • If you are requesting for a specific department, division, or location, please let us know.
  • Our policy is to prioritize requests where facilities explicitly say they will allow staff members and volunteers to use our donated materials. If your facility explicitly prohibits them, we will not be able to fulfill your request.
  • Items Requested

    Please let us know the quantities for each item requested for your staff. We ask our craftivists to donate handmade masks with two tightly woven layers of 100% cotton. For more details about items, please review our patterns page at: millionmaskchallenge.com/information
  • (Our ability to donate will depend on what we have received from craftivists.)
  • Larger face masks with ties, designed to cover the exterior of a medical N95 respirator
  • If available, would you like 3D-printed "ear savers" to use with elastic ear loops on masks?
  • Please tell us about your organizational color requirements, needs for certain kinds of ties or elastic, or other projects and needs. We cannot meet every request, but the information is helpful to understand your needs. If you have another need that is not one of our current patterns, you can also tell us about your idea.
  • Specific colors that may be required for police, military, first responders, etc.
  • Delivery Information

    For the safety of our volunteers, we need to confirm how we will safely deliver your donated items.
  • Let us know if you think this will be someone else or yourself.
  • We need a cell phone number for someone who can be responsible for receiving the donation, and permission to text that person to coordinate.
  • How would you prefer to receive your donated masks, caps, or other items?
  • Please give us the specific delivery address for the requested donation, if different from the primary business address.
  • Please let us know any other specific delivery instructions, such as who we call upon arrival for delivery, the days and times when delivery is possible, and for mailings, to whom the package should be addressed.
  • May we share the name of your institution publicly or via Facebook? You may opt out, but if we can't post a sign-up for volunteers to sew for you, it may be MORE DIFFICULT to engage volunteers to fulfill this request.
  • Million Mask Challenge will use the information you provide to stay in touch and provide you with additional instructions about how to receive a donation from us. By submitting this form, you give us permission to contact you by email with basic instructions. You will receive an additional opt-in query to receive additional a very infrequent newsletter via our Mail Chimp list. You may accept, decline or change your mind at any time; simply click the unsubscribe link in an email newsletter or contact us at info@millionmaskchallenge.com to be removed from our contact list.
